A provider–patient relationship is established only through a separate membership agreement and the commencement of clinical care. Membership Services
Information on the Site regarding our concierge membership, services, and availability is provided for general informational purposes and does not constitute an offer or a guarantee of service, availability, or specific outcomes. Membership is subject to a separate written membership agreement, applicable eligibility criteria, and panel availability. In the event of any conflict between the Site and a signed membership agreement, the membership agreement controls.
