Services

Membership covers the whole of your primary care: preventive, chronic, and acute. Care is delivered wherever you happen to be, for as long as each visit actually requires.

Primary & Preventive Care

Comprehensive primary care, with the time to practice it properly.

Most of what makes primary care valuable is not procedural. It is the accumulated knowledge of one person's body over years: what your baseline looks like, which numbers have been quietly drifting, what you mentioned last spring and never came back to.

That kind of medicine is not difficult. It is simply impossible to deliver in twelve minutes with two thousand other patients on the panel.

With ninety minutes and a capped panel, an annual evaluation becomes a genuine review rather than a checklist: a complete history, a thorough physical, and labs read against your own prior results rather than a population range. Chronic conditions are managed between visits instead of only at them. And when something acute happens, you are not triaged into the next available opening three weeks out. You are seen.

It also includes the care people are least able to arrange for themselves. Anxiety and depression are treated here as ordinary parts of primary care: discreetly, in your own home, by a provider who already knows your history. No separate referral, no waiting list, no unfamiliar office, and no explaining yourself from the beginning to someone new.

Children are welcome. Each member of a household holds their own place on the panel and their own relationship with the provider: a child is a patient here in their own right, not an entry in a parent's chart.

  • Comprehensive annual evaluation and physical
  • Chronic condition management: blood pressure, cholesterol, diabetes, thyroid
  • Acute illness and injury, seen the same day
  • Mental health care, including anxiety and depression
  • Care for children and adolescents
  • Minor procedures performed in your home
  • Specialist referral, coordination, and follow-through

House-Call & Mobile Delivery

Care delivered at your home, your office, or your boat.

Latitude 26 has no waiting room, because there is no office for you to visit.

Visits happen where you are. For most members that means home. For some it means an office between meetings, or a boat at the dock. The clinical bag carries what a well-equipped exam room carries. What it does not carry is the hour of your day spent driving, parking, waiting, and completing a form you have completed before.

Discretion is built into the model rather than offered as an accommodation. Arrivals are unmarked and unremarkable. Nothing about a visit announces itself to a neighbor, a doorman, or a colleague.

  • Unlimited house calls within the service area, when clinically indicated
  • Visits at your office, by arrangement
  • Dockside and onboard visits
  • Unmarked arrival, no waiting room
  • Travel beyond the service area by arrangement

In-Home Diagnostics & Labs

Diagnostics and laboratory work performed where you are.

Blood is drawn at your kitchen table rather than in a queue at a draw station.

What happens in the next two hours matters as much. Several common panels, among them metabolic, lipid, and potassium, begin to drift the moment a sample sits. Latitude 26 processes samples on site rather than carrying whole blood across the county first, so what the laboratory measures is closer to what was actually in your bloodstream.

Point-of-care testing answers some questions during the visit itself. Where imaging is needed, it is arranged and pursued on your behalf. You are not the one telephoning a radiology department.

Results come back as a conversation. Not a portal notification with a red flag and no explanation attached.

  • Phlebotomy performed in your home
  • Samples processed on site for accuracy
  • Point-of-care testing with same-visit results
  • Imaging arranged and coordinated
  • Every result reviewed with your provider directly

Membership Add-Ons

Elective services, available to members by arrangement.

A small number of elective services sit outside the membership. They are optional by design. Nothing here is necessary to your care, and nothing about the membership assumes you will want any of it.

Where a member does, each is arranged individually and priced separately, and discussed the same way the retainer is: in conversation rather than from a menu.

Each is provided under the same clinical standard as the rest of your care. Where a treatment is elective, that is said plainly. Where the evidence behind it is limited, that is said plainly too.

  • Intravenous hydration and nutrient therapy
  • Medically supervised weight management
  • Advanced longevity and risk profiling

Membership limited to 100 patients per provider.
